Scheduling an Appointment:

Please note that each patient has their own unique username and password. If you have lost track of yours please email me for a new one. Once you are signed in, select the tab called "appointments" and follow the instructions from there.
If you are unable to find a time in the schedule that works for you, or is soon enough for you, please send me an email or leave me a phone message. If your concern is urgent, please call me on my cell phone.
If need be, you may also call or email me and I will schedule your appointment for you.
Walk-in Time:
You can walk in without an appointment between 7:30 AM and 8:30AM Monday to Friday. Please be aware that during walk in time, there may not be time for a long discussion, addressing multiple issues or for figuring out complicated problems. If we need more time, we will set up a follow up time to finish our work.

What to bring to your appointment:
- your insurance card
- your copay or payment - see FAQs regarding payment at the time of service
- any forms you would like me to complete
- all of your medicine bottles including over the counter medicines, herbs and supplements or a detailed list with all brand names, active ingredients and doses.
- a list of your concerns or questions.
- a friend or family member if you would like their support.
What to do before a visit:
Please arrive at my office a few minutes before your visit so you have time to complete a pre - visit form that will help us plan your visit. This information alerts me to focus our visit on what is important to you.
For a adult wellness visit:
You will find a rack of forms on the counter. Please select a man's or woman's form. After you have completed the form, please proceed to the next room and take your blood pressure and pulse and weight. If you have emailed me a How's Your Health check up report then you do not need to complete a pre-visit form but do please take your vital signs.
For a well child visit: You will find a rack of forms on the counter. Please select the form that matches your child's age. After you have completed the form, please proceed to the next room and take child's your blood pressure and weight if you feel comfortable doing so.
For a follow up visit or a visit for a new problem: Please go directly into the next room to take you blood pressure, pulse, height and weight. On the table there is a form called "Visit Planner". Please record your numbers there and fill out the other side of the form.
