At this time, I am only accepting newborn babies and family members on a limited basis. I am not accepting other new patients. If you want to be placed on my waiting list, please see the instructions at the bottom of this page.

especially the Policies page.Please email me at averywoodmd@onebox.com with
any questions.
After reviewing this material, please complete the Signature Page, Registration form and the Records Release form. These three forms can be downloaded and printed from the Forms page. (A records release is not needed for newborns.)
Please contact your insurance company to be sure I am listed as
your or your child's primary care doctor. If I am not, your insurance may not cover
your visit and you will be responsible for paying.
When I receive
your registration form, I will send you a password for scheduling an
appointment on this web site. For more information on scheduling click on Scheduling.
In the future I hope to be accepting new patients. If you would like to be notified when this
happens, please send me an email averywoodmd@onebox.com.

1) Your name and address 2) The names of other family members
registering
3) Everyone's birthday,
4) Your health insurance 5) Any questions or concerns you may
have.
6)Please write "waiting list" in the subject line of your
