You can walk in without an appointment between 7:30 AM and 8:30AM. This time can be used for a new problem like a rash, an ear infection or a twisted ankle. Walk-in time can also be used to recheck a stable old problem.  At a walk in time, there may not be time for a long discussion, addressing multiple issues or for figuring out complicated problems. In this case, we will set up a follow up time to finish our work.

You will be asked to give your username and password.  These should have been emailed to you after you sent in your registration papers. Please send me an email if you have not received them.  Please be sure to click the "Make Appointment" button  at the end or your appointment will be lost.  Your appointments can be verified under the " My Appointments" tab. 

You should receive an email confirmation of your appointment right away. Shortly before your appointment you will receive a reminder email.  Please let me know if your email address needs to be updated or if you do not want to receive emails about your appointments.

If you are unable to find a time that works for you please send me an email or leave me a phone message.  If your concern is urgent, please page me.


Please bring your insurance card and any forms you would like me to complete. Please bring your copay. Please be prepared to pay at the time of your visit. When you pay at the time of your visit you will receive a 25% discount on payments other than co pays.

Please also bring all your medication bottles or an up to date medication list that includes dosages.  Please include include over the counter medicines, herbs and supplements.


In the future, when you schedule your appointment you may be asked to fill out a pre-visit questionnaire from Hows your Health or Instant Medical History. These tools help alert me to problems you may be having. They help us focus on what is important to you. I intend to make these available to you on line.  For the time being, we'll use paper forms after you arrive at the office.
